Complete Intake Paperwork
24 hours before your appointment.
Fill out a release of information for collaboration with your care team.
Make a list of questions and concerns to ask your provider in your appointment.
Meet Your Provider
The first time you meet with your provider will include an initial assessment. You can expect it to be 50-60 minutes.
Arrive 15 minutes before your appointment time to check-in.
Your provider will get to know you and understand your experiences.
You can expect to receive education and together, make a treatment plan.
